What a UK fit note can say
The official Statement of Fitness for Work records that you are either not fit for work or may be fit for work taking account of advice. It does not include a “fit for work” option. If your employer asks for proof that you are fully fit to return, ask what document or occupational-health assessment they require before booking.
For Statutory Sick Pay, an employer can normally ask for a fit note only after more than 7 calendar days of sickness. For the first 7 days, you normally self-certify. Employer policies can differ for other purposes.
A fit note may cover an earlier period only if the clinician considers that medically appropriate from the assessment and available evidence. Dates and wording are clinical decisions, not choices guaranteed by this form.

Important questions
Who can legally issue a fit note?
Eligible registered doctors, nurses, occupational therapists, pharmacists and physiotherapists can issue one when working within their competence and after the required assessment. Availability on Time Medics may be narrower.
Is a fit note guaranteed?
No. The clinician must assess how your health affects your fitness for work and remains responsible for the decision.
Will my employer accept it?
A fit note is official medical evidence, but employers make employment decisions and may discuss adjustments with you. Ask your employer in advance if they require a particular process.
Does this cover Northern Ireland?
Northern Ireland has its own mirrored fit-note legislation. The assessment is still required and fit notes are generally used after the first 7 days. Your clinician must be appropriately registered and able to issue the relevant document for your location.
